Patients are seen by appointment only.
 

  • All new patients will be scheduled to see one of our physicians within 7-10 days.

  • Follow-up appointments are scheduled at your convenience.

  • A nurse is available during regular office hours to take phone calls with any medical concerns.

  • If you are unable to keep your appointment, we ask for 48-hour advance notice of your cancellation.


Please allow yourself time to complete our registration forms at the time of your first visit to our office.

You can print them on your home printer from this website and complete them in advance of your appointment and bring them with you. Forms are available at the end of this page.


Do I need a physician referral to make an appointment?

We prefer that your physician refer you since she or he could best assess your need for a gynecologic oncologist; however, if you have a known gynecologic cancer, you are welcome to call us directly and we would be happy to see you.
 

  • It is our policy to obtain all pertinent medical records, including but not limited to, pap tests, biopsy results, imaging, laboratory and office notes from your OB/GYN physician.
  • Referral and follow-up communication is coordinated with your OB/GYN physician.
  • Your insurance company may require you to have a referral from your primary care or regular OB/GYN physician to be seen in our practice. Please verify your insurance requirements prior to your first visit to our office.


What should I bring to my appointment?

We ask that you bring:

  • Current insurance card
  • Photo ID
  • List of current medications
  • List of any known allergies including reactions
  • List of previous surgeries


Please be prepared to discuss your family and personal health history. We also ask that you come prepared to pay any co-pays, deductibles or non-covered service fees as required by your insurance carrier.
